For our purpose, we can make a Service Field where customers can choose what kind of website they want you to design with the help of Drop Down Field

You can give a range where the customers can choose how many design mockups they want. And you can set a unit price for each mockup.
After that, if you also design for mobile devices and want to keep an option for the customers to add mobile design mockups, mention it with a Radio Field.
If a customer wants a mobile mockup they can choose the yes option to know the price unit. Otherwise a no option would mean that a desktop version will suffice.
You can add HTML Lines of your own,
Afterwards, if you offer more services you can add them using Checkbox Fields.
You can add as many fields you want. The customer can check the box if they want the services.

You can customize the background color, shape and fonts.
After getting the whole cost estimation of the design needs, the customer will submit a form that will be emailed to you.
