If a customer wants a mobile mockup they can choose the yes option to know the price unit. Otherwise a no option would mean that a desktop version will suffice.

You can add HTML Lines of your own,

Afterwards, if you offer more services you can add them using Checkbox Fields.

You can add as many fields you want. The customer can check the box if they want the services.
